Terms and Conditions
This contract is between (1) Ultrasound Scan Clinic (“we”, “us”) and (2) you, the Client (“you”), and shall be governed by the following terms and conditions of sale (“Conditions”), none of which affect your statutory rights:
1. SCAN SERVICES
We provide pregnancy ultrasound scans including early pregnancy, reassurance, dating, gender, anomaly, 3D and 4D.
We provide general scans including, upper abdominal, renal and testes scans.
We accept self-referrals and those from medical professionals.
All scans are completed by a trained healthcare professional who is registered with the Health and Care Professions Council (HCPC), CORU and a member of the Society of Radiographers (SoR) and British Medical Ultrasound Society (BMUS). They have previous experience working in both the National Health Service (NHS) and private ultrasound clinics. If another sonographer is employed to perform ultrasound scans at Ultrasound Scan Clinic, they will be registered with the Health and Care Professions Council and have sufficient qualifications, skills and experience to perform scans to the same high standard.
For pregnancy scans, the health and wellbeing of your baby will be checked at every scan. Depending on what type of scan you have booked, we will check the size of your baby, check for multiple pregnancies, show the position of the baby and the placenta. If we detect any problems with your baby’s development, we will discuss this with you. If necessary we will direct you to an NHS care provider for further investigation.
For general ultrasound scans, we will undertake the scan that is requested based on the clinical history you have provided. We will create a report for every examination undertaken and provide this to you digitally. You can forward this on to the relevant medical professional(s) if required. If we deem the selected scan is not suitable after clinical history is taken, you will be informed of this and advised to contact your medical provider.
2. MEDICAL HISTORY
At each scan, we will ask you your relevant medical history. However, we do not have access to your full medical history and therefore cannot provide medical advice relating to any diagnosis. We will always direct you back to your relevant medical professional(s).
For pregnancy ultrasound scans, we strongly recommend that you access all antenatal services made available to you by the NHS.
3. METHOD
We aim to complete pregnancy scans abdominally but we may need to undertake an internal (transvaginal) scan. You have every right to decline an internal scan. If you decline an internal scan, however, you accept that we may not be able to provide you with conclusive information. We will discuss this decision with you at the time. Should an internal ultrasound scan be necessary and you are happy to have this done, we will ask you to read and sign a consent form before doing so.
4. MODIFICATION
We reserve the right to modify or improve our service without prior notice.
5. PHOTOGRAPHY & RECORDING
Please ask for permission before any photography or recording is carried out in our clinic. You will receive printed images at every pregnancy scan and sometimes video clips depending on the chosen ultrasound scan.
6. AGE
You must be an adult (18 years or older). We do not extend our scan services to anyone under the age of 18 years old.
7. RIGHT TO MAKE CHANGES
If you wish to make a change to the chosen service, please contact us. We will let you know whether the change is possible.
8. PRICE & PAYMENTS
We reserve the right to modify our prices at any time and without prior notice. We accept payment on debit or credit cards including Visa and Mastercard. We accept cash payments. When booking online, a deposit of £30 will be required to secure your appointment booking.
9. INFORMATION WE REQUIRE TO PROVIDE SERVICES
We will need certain information from you to provide our services. We will ask you verbally (or using equivalent methods) for relevant information/history to assist with our examination. On occasion we may need you to complete a short form prior or on arrival to our clinic. This information will be primarily identity based and necessary, relevant medical information only.
10. CANCELLATION OF THE SERVICES BY YOU
If you wish to cancel your appointment, we request a minimum of a 48-hour notice period prior to your scheduled appointment. This can be done via phone call or email. You will need to contact our clinic to process the £30 deposit paid if you have cancelled within the appropriate timeframe set out above .
If you do not attend your scheduled appointment, we reserve the right to hold your £30 deposit.
Re-scheduling is permitted. This can be done via phone call or email but must be done within a 24-hour notice period of scheduled appointment time.
We may, at our discretion, offer additional service guarantees outside of these terms and conditions, for which we reserve the right to modify without prior notice.
11. CANCELLATION OF THE SERVICES BY US
We may cancel or re-schedule your appointment.
Such cancellation or change would only occur in emergency.
If this occurs, we will give you as much notice as possible and re-schedule your appointment as soon as possible. In such cases if rescheduling is not suitable for you, we will offer a full refund of your deposit taken.
12. WHAT YOU WILL RECEIVE FOLLOWING THE SCAN
After your ultrasound scan you will receive a digital scan report and any advertised package deliverables in the form of printed and electronic images and or video clips.
13. OWNERSHIP OF THE PRODUCT
We retain copyright of any images that we take. Ownership of the report shall belong to you. Images (Non-obstetric ultrasound scans) can be requested if necessary for relevant medical practitioner(s).
14. WHAT WE CANNOT GUARANTEE
We endeavour to provide you with the best quality images/examination at the time of your ultrasound scan however this can be affected by a number of factors. This includes but is not exhaustive of, your body habitus and body mass index (BMI), close proximity or overlaying bowel gas, baby’s position during a pregnancy related scan, position of placenta during a pregnancy related scan, amniotic fluid levels and abdominal scarring including previous caesarean sections during a pregnancy related scan. With regards to pregnancy related ultrasound scans, if we cannot provide the purpose of the scan at the time of your scan due to your baby’s positioning, you may be offered one follow up scan (at no additional cost) in an appropriate time frame.
For gender scans, we will do our best to identify the sex of your baby but we cannot guarantee 100% accuracy. This can be affected by baby’s position and other factors outlined above. We accept no liability for any incorrect indication of your baby’s gender.
15. OUR LIABILITY TO YOU
We shall not be responsible or liable for any injury, loss or damage to personal belongings whilst you are in our premises. We are only liable for our failure to comply with these Conditions and accept responsibility for loss or damage you suffer that is a foreseeable result of our breaking this contract or our failing to use reasonable care and skill, but we are not responsible for any loss or damage that is not foreseeable or not caused by us breaking the contract.
16. DELAYS OUTSIDE OUR CONTROL
If we have to delay or amend the date or time of your appointment due to an event outside our control then we will contact you as soon as possible to let you know. For example, if we have technical issues or changes in law or regulatory requirements. We will not be liable for delay.
17. HOW WE USE YOUR DATA
When you attend for your appointment or when booking online, you will be asked to complete a form to ascertain some identity, personal and contact data. This will be used to provide our services and to create and provide your report. We will only share information with medical professionals with your consent or when we are legally bound to by governing law.
Consent and understanding of privacy policy will be assumed when you interact or continue with our website, booking system, payment system and services. By using Ultrasound Scan Clinic services you consent to the use of third-party providers such as Acuity scheduling, Stripe and Square payment sytems and Trice medical imaging archiving and distributing services.
